Do I need a referral to make an appointment?
Some insurances require a referral to see a specialist, such as an HMO. A referral may not be required if you have a PPO. Check with your insurance company to see if a referral is necessary and if our practice if in network for your specific plan.
What should I bring for an initial consultation?

Please bring the following to your appointment:

  • Driver’s License or a valid ID
  • Insurance card
  • Referral (if required based on your insurance plan)
  • Medical records, including X-rays, MRI’s, CT scans or other relevant information
  • List of medications you are currently taking

We encourage you to come to your initial consultation with a written list of questions to ensure you don’t forget to ask them when you are seeing the doctor.

Are my medical records kept private and confidential?
Your medical file is handled with the utmost respect for your privacy. Our staff is bound by strict confidentiality requirements as a condition of employment regarding your medical records. We will not release the contents of your medical file without your consent.
How do I contact Premier Allergy after hours?

In case of an emergency, please dial 9-1-1 or go to the nearest emergency room or urgent care. You can listen to the full after-hours voice message for more information on how to speak with someone after hours. Please note, a fee will be incurred for any medical advice given over the phone.

How long should I expect for my first appointment?
Initial visits are expected to be between 2-3 hours in length. Please schedule accordingly.
What should I expect at my first appointment?
Initial visits will consist of a full history, physical exam and visit with a provider. A variety of testing and treatment (spirometry, breathing treatments, skin testing) may be performed. Medication demonstration, additional testing, labs or referrals may be a portion of your visit, depending on your case.
Does the allergy testing involve needles?
We test for a variety of allergens that you may be allergic to. Most of our allergy testing is needle-free! It is typically placed on your back and upper arms to help minimize discomfort.

How much will my visit with an allergist cost?

As a courtesy, we verify your benefits prior to your appointment and will call you with an estimate. Any information provided is just an estimate, not a guarantee of payment. We encourage you to call your insurance company as well to verify that information. Benefits for a specialist can be different than the benefits with your primary care doctor. You can reach out to our office with any additional questions you may have.

Do you offer same day appointments?
Yes. This is schedule dependent.
Do you have a cancellation fee?
We do enforce a no-show or same-day cancellation fee in order to respect patient and provider scheduling.
What forms of payment do you accept?
We do not accept cash or checks at our offices. All major credit cards accepted.
